Surname: Last name entry must be at least 3 letters long and should only contain letters. Form Given: This must also be filled out with at least 3 letters. Form Middle: Enter a middle name or initial here if applicable. Form Maiden: Enter maiden or birth name here if applicable. Form Death Date: Write out the death date by spelling out full month names. Do not use a number for the month. Use the full year such as 1902 not 02. Please refrain from about, before, after or circa dating. If you have a source other than word of mouth for the record you won't need to use this type of date entry. Form Select The County: Pick a county for your record from the drop down list. If you do not know the county your homework on this person is not done, and the record may not be accepted. Form Source or Notes: Required! Please answer respectfully with words containing 10 letters and spaces or more. The ENTER KEY is disabled in this area due to the problems it causes in databases.
